Ethical questions science can’t answer

Yes, yes, we’ve covered this territory before. But you might have heard that Sam Harris has reopened the discussion by challenging his critics, luring them out of their hiding places with the offer of cold hard cash. You see, even though Sam has received plenty of devastating criticism in print and other venues for the thesis he presents in The Moral Landscape (roughly: there is no distinction between facts and values, hence science is the way to answer moral questions), he is — not surprisingly — unconvinced. Hence the somewhat gimmicky challenge. We’ll see how that ones goes, I already have my entry ready (but the submission period doesn’t open until February 2nd).

Be that as it may, I’d like to engage my own thoughtful readers with a different type of challenge (sorry, no cash!), one from which I hope we can all learn something as the discussion unfolds. It seems to me pretty obvious (but I could be wrong) that there are plenty of ethical issues that simply cannot be settled by science, so I’m going to give a few examples below and ask all of you to: a) provide more and/or b) argue that I am mistaken, and that these questions really can be answered scientifically.

Who's Afraid of Peer Review?

On 4 July, good news arrived in the inbox of Ocorrafoo Cobange, a biologist at the Wassee Institute of Medicine in Asmara. It was the official letter of acceptance for a paper he had submitted 2 months earlier to the Journal of Natural Pharmaceuticals, describing the anticancer properties of a chemical that Cobange had extracted from a lichen.

In fact, it should have been promptly rejected. Any reviewer with more than a high-school knowledge of chemistry and the ability to understand a basic data plot should have spotted the paper's short-comings immediately. Its experiments are so hopelessly flawed that the results are meaningless.

I know because I wrote the paper.

10 ways to create “False Knowledge” in Psychology

There’s been quite a good deal of discussion recently about (1) how we validate a scientific fact (herehere and here), and (2) whether psychology – and in particular some branches of psychology – are prone to generate fallacious scientific knowledge (here and here). As psychologists, we are all trained (I hope) to be scientists – exploring the boundaries of knowledge and trying as best we can’ to create new knowledge, but in many of our attempts to pursue our careers and pay the mortgage, are we badly prone to creating false knowledge? Yes – we probably are! Here are just a few examples, and I challenge most of you psychology researchers who read this post to say you haven’t been a culprit in at least one of these processes!

Here are 10 ways to risk creating false knowledge in psychology.

1.  Create your own psychological construct. Constructs can be very useful ways of summarizing and formalizing unobservable psychological processes, but researchers who invent constructs need to know a lot about the scientific process, make sure they don’t create circular arguments, and must be in touch with other psychological research that is relevant to the understanding they are trying to create.

That is the conclusion of a study published Thursday in the journal Science. It found that after reading literary fiction, as opposed to popular fiction or serious nonfiction, people performed better on tests measuring empathy, social perception and emotional intelligence — skills that come in especially handy when you are trying to read someone’s body language or gauge what they might be thinking.

The researchers say the reason is that literary fiction often leaves more to the imagination, encouraging readers to make inferences about characters and be sensitive to emotional nuance and complexity.

Book Review: 'Behind the Shock Machine' by Gina Perry

To almost everyone's surprise at the time, upward of two-thirds of the participant-teachers administered what they thought were the highest levels of shock, even though many were sweating and suffering over the pain they believed they were inflicting on a stranger in the name of science. Milgram's experiment produced a firestorm of protest about the potential psychological harm inflicted on the unwitting participants. As a result, it could never be done today in its original version.

Some people hated the method and others the message, but the Milgram study has never faded from public attention. It has been endlessly retold in schoolrooms, textbooks, TV programs, novels, songs and films. What, then, is left to say about it?

According to Gina Perry, an Australian psychologist and journalist, everything. She has investigated every aspect of the research and spoken with seemingly anyone who had a connection to Milgram (1933-84). She describes each of Milgram's 24 experimental variations on the basic obedience paradigm. She interviewed some of the original subjects, the son of the man who played the "learner," Milgram's research assistants, his colleagues and students, his critics and defenders, and his biographer. She listened to audiotapes of the participants made during and after the experiments. She pored through the archives of Milgram's voluminous unpublished papers.

Intent to Harm: Willful Acts Seem More Damaging

How harmful we perceive an act to be depends on whether we see the act as intentional, reveals new research published in Psychological Science, a journal of the Association for Psychological Science.

The new research shows that people significantly overestimate the monetary cost of intentional harm, even when they are given a financial incentive to be accurate.

"The law already recognizes intentional harm as more wrong than unintentional harm," explain researchers Daniel Ames and Susan Fiske of Princeton University. "But it assumes that people can assess compensatory damages -- what it would cost to make a person 'whole' again -- independently of punitive damages."

According to Ames and Fiske, the new research suggests that this separation may not be psychologically plausible:

"These studies suggest that people might not only penalize intentional harm more, but actually perceive it as intrinsically more damaging."

Recent Findings Force Scientists To Rethink The Rules Of Neuroimaging

Brain mapping experiments all share a basic logic. In the simplest type of experiment, researchers compare brain activity while participants perform an experimental task and a control task. The experimental task might involve showing participants a noun, such as the word "cake," and asking them to say aloud a verb that goes with that noun, for instance "eat." The control task might involve asking participants to simply say the word they see aloud.

"The idea here is that the control task involves some of the same cognitive processes as the experimental task, in this case perceptual and articulatory processes," Jack explained. "But there is at least one process that is different - the act of selecting a semantically appropriate word from a different lexical category."

The Morality of Meditation

Gaining competitive advantage on exams and increasing creativity in business weren’t of the utmost concern to Buddha and other early meditation teachers. As Buddha himself said, “I teach one thing and one only: that is, suffering and the end of suffering.” For Buddha, as for many modern spiritual leaders, the goal of meditation was as simple as that. The heightened control of the mind that meditation offers was supposed to help its practitioners see the world in a new and more compassionate way, allowing them to break free from the categorizations (us/them, self/other) that commonly divide people from one another.

But does meditation work as promised? Is its originally intended effect — the reduction of suffering — empirically demonstrable?

To put the question to the test, my lab, led in this work by the psychologist Paul Condon, joined with the neuroscientist Gaëlle Desbordes and the Buddhist lama Willa Miller to conduct an experiment whose publication is forthcoming in the journal Psychological Science.

The Suicide Detective

Despite the progress made by science, medicine and mental-health care in the 20th century — the sequencing of our genome, the advent of antidepressants, the reconsidering of asylums and lobotomies — nothing has been able to drive down the suicide rate in the general population. In the United States, it has held relatively steady since 1942. Worldwide, roughly one million people kill themselves every year. Last year, more active-duty U.S. soldiers killed themselves than died in combat; their suicide rate has been rising since 2004. Last month, the Centers for Disease Control and Prevention announced that the suicide rate among middle-aged Americans has climbed nearly 30 percent since 1999.

Trying to study what people are thinking before they try to kill themselves is like trying to examine a shadow with a flashlight: the minute you spotlight it, it disappears. Researchers can’t ethically induce suicidal thinking in the lab and watch it develop. Uniquely human, it can’t be observed in other species. And it is impossible to interview anyone who has died by suicide. To understand it, psychologists have most often employed two frustratingly imprecise methods: they have investigated the lives of people who have killed themselves, and any notes that may have been left behind, looking for clues to what their thinking might have been, or they have asked people who have attempted suicide to describe their thought processes — though their mental states may differ from those of people whose attempts were lethal and their recollections may be incomplete or inaccurate. Such investigative methods can generate useful statistics and hypotheses about how a suicidal impulse might start and how it travels from thought to action, but that’s not the same as objective evidence about how it unfolds in real time.

Breaking the Seal on Drug Research

For years, researchers have talked about the problem of publication bias, or selectively publishing results of trials. Concern about such bias gathered force in the 1990s and early 2000s, when researchers documented how, time and again, positive results were published while negative ones were not. Taken together, studies have shown that results of only about half of clinical trials make their way into medical journals.

Problems with data about high-profile drugs have led to scandals over the past decade, like one involving contentions that the number of heart attacks was underreported in research about the painkiller Vioxx. Another involved accusations of misleading data about links between the antidepressant Paxil and the risk of suicide among teenagers.

To those who have followed this issue for years, the moves toward openness are unfolding with surprising speed.

“This problem has been very well documented for at least three decades now in medicine, with no substantive fix,” said Dr. Ben Goldacre, a British author and an ally of Dr. Doshi. “Things have changed almost unimaginably fast over the past six months.”

Much of that change is happening because of what Dr. Goldacre calls an “accident of history.” In 2009, Dr. Doshi and his colleagues set out to answer a simple question about the anti-flu drug Tamiflu: Does it work? Resolving that question has been far harder than they ever envisioned, and, four years later, there is still no definitive answer. But the quest to determine Tamiflu’s efficacy transformed Dr. Doshi and others into activists for transparency — and turned the tables on drug makers. Until recently, the idea that companies should routinely hand over detailed data about their clinical trials might have sounded far-fetched. Now, the onus is on the industry to explain why it shouldn’t.

How Reading Makes Us More Human

A debate has erupted over whether reading fiction makes human beings more moral. But what if its real value consists in something even more fundamental?

A battle over books has erupted recently on the pages of The New York Times and Time. The opening salvo was Gregory Currie's essay, "Does Great Literature Make Us Better?" which asserts that the widely held belief that reading makes us more moral has little support. In response, Annie Murphy Paul weighed in with "Reading Literature Makes Us Smarter and Nicer." Her argument is that "deep reading," the kind of reading great literature requires, is a distinctive cognitive activity that contributes to our ability to empathize with others; it therefore can, in fact, makes us "smarter and nicer," among other things. Yet these essays aren't so much coming to different conclusions as considering different questions.

To advance her thesis, Paul cites studies by Raymond Mar, a psychologist at York University in Canada, and Keith Oatley, a professor emeritus of cognitive psychology at the University of Toronto. Taken together, their findings suggest that those "who often read fiction appear to be better able to understand other people, empathize with them and view the world from their perspective." It's the kind of thing writer Joyce Carol Oates is talking about when she says, "Reading is the sole means by which we slip, involuntarily, often helplessly, into another's skin, another's voice, another's soul."

Oatley and Mar's conclusions are supported, Paul argues, by recent studies in neuroscience, psychology, and cognitive science. This research shows that "deep reading -- slow, immersive, rich in sensory detail and emotional and moral complexity -- is a distinctive experience," a kind of reading that differs in kind and quality from "the mere decoding of words" that constitutes a good deal of what passes for reading today, particularly for too many of our students in too many of our schools (as I have previously written about here).

Paul concludes her essay with a reference to the literary critic Frank Kermode, who famously distinguishes between "carnal reading" -- characterized by the hurried, utilitarian information processing that constitutes the bulk of our daily reading diet -- and "spiritual reading," reading done with focused attention for pleasure, reflection, analysis, and growth. It is in this distinction that we find the real difference between the warring factions in what might be a chicken-or-egg scenario: Does great literature make people better, or are good people drawn to reading great literature?

Person as Scientist, Person as Moralist

It has often been suggested that people’s ordinary capacities for folk psychology and causal cognition make use of much the same methods one might find in a formal scientific investigation. A series of recent experimental results offer a challenge to this widely-held view, suggesting that people’s moral judgments can influence the intuitions they hold both in folk psychology and in moral cognition. The present target article argues that these effects are best explained on a model according to which moral considerations actually figure in the fundamental competencies people use to make sense of the world.

Consider the way research is conducted in a typical modern university. There are departments for theology, drama, philosophy… and then there are departments specifically devoted to the practice of science. Faculty members in these science departments generally have quite specific responsibilities. They are not supposed to make use of all the various methods and approaches one finds in other parts of the university.

Now consider the way the human mind ordinarily makes sense of the world. One plausible view would be that the human mind works something like a modern university.   There are psychological processes devoted to religion (the mind’s theology department), to aesthetics (the mind’s art department), to morality (the mind’s philosophy department) … and then there are processes specifically devoted to questions that have a roughly ‘scientific’ character. These processes work quite differently from the ones we use in thinking about, say, moral or aesthetic questions. They proceed using more or less the
same sorts of methods we find in university science departments.

This metaphor is a powerful one, and it has shaped research programs in many different areas of cognitive science. Take the study of folk psychology. Ordinary people have a capacity to ascribe mental states (beliefs, desires, etc.), and researchers have sometimes suggested that people acquire this capacity in much the same way that scientists develop theoretical frameworks (e.g., Gopnik & Wellman 1992). Or take causal cognition. Ordinary people have an ability to determine whether one event caused another, and it has been suggested that they do so by looking at the same sorts of statistical information scientists normally consult (e.g., Kelley 1967). Numerous other fields have taken a similar path. In each case, the basic strategy is to look at the methods used by professional research scientists and then to hypothesize that people actually use similar methods in their ordinary understanding. This strategy has clearly led to many important advances.

Biological psychiatry’s false paradigm—still no proof mental illness is a biological disease

Days before the official May 22 publication date of the “Diagnostic and Statistical Manual of Mental Disorders” (DSM-5), a number of psychiatrists who were closely associated with the project scrambled to do some preemptory damage control, mostly by lowering the expectations for what was to come.

Michael B. First, professor of psychiatry at Columbia, acknowledged on NPR that there was still no empirical method to confirm or rule out any mental illness. “We were hoping and imagining that research would advance at a pace that laboratory tests would have come out. And here we are 20 years later and we still unfortunately rely primarily on symptoms to make our diagnoses.” Speaking to The New York Times, Thomas R. Insel, director of the National Institutes of Mental Health, insisted that this failure had not been for lack of effort.

In the same Times article, David J. Kupfer, chairman of the DSM-5 Task Force, admitted “a failure of our neuroscience and biology to give us the level of diagnostic criteria, a level of sensitivity and specificity that we would be able to introduce into the diagnostic manual.” Drs. Kupfer, Insel and First agree that the new paradigm envisioned for psychiatry — the reason the new edition was undertaken — remains elusive.

Book raises alarms about alternative medicine

Doctors diagnosed her with acute pancreatitis, in which pancreatic enzymes begin digesting not just food, but the pancreas itself.

The most likely cause of the girl's condition: toxic side effects from more than 80 dietary supplements, which the girl's mother carried in a shopping bag, says Sarah Erush, clinical pharmacy manager at Children's Hospital of Philadelphia, where the girl was treated last summer.

The girl's mother had been treating her with the supplements and other therapies for four years to treat the girl's "chronic Lyme disease," a condition that, experts say, doesn't actually exist. While some Lyme infections cause pain and other lingering symptoms, the infections don't persist for years. And, according to the Infectious Disease Society of America, the infections don't require years of antibiotics or other risky therapies given by some alternative medicine practitioners.

Doctors were able to control the girl's illness with standard therapies, Erush says, and she was discharged from the hospital after two weeks.

Although the child's story was unforgettable, Erush says, it wasn't unusual. Parents now "routinely" bring children to her hospital with a variety of alternative remedies, hoping that nurses will administer them during a child's stay.

Arthur Caplan, the director of medical ethics at NYU Langone Medical Center in New York, says alternative healers satisfy patients' needs for more personal attention.

"Medicine does a very poor job of addressing the emotional, spiritual and even psychological side of things," Caplan says. "When you are not good at doing important things, other people rush into that vacuum."

Yet people who put their faith in alternative healers and supplements may be putting themselves at risk, Caplan says.

The alternative therapy industry capitalizes on a number of common sentiments, Offit says, from a naïve belief in the safety of all things natural to distrust of government regulation.

The Mind of a Con Man

Stapel was an academic star in the Netherlands and abroad, the author of several well-regarded studies on human attitudes and behavior. That spring, he published a widely publicized study in Science about an experiment done at the Utrecht train station showing that a trash-filled environment tended to bring out racist tendencies in individuals. And just days earlier, he received more media attention for a study indicating that eating meat made people selfish and less social.

His enemies were targeting him because of changes he initiated as dean, Stapel replied, quoting a Dutch proverb about high trees catching a lot of wind. When Zeelenberg challenged him with specifics — to explain why certain facts and figures he reported in different studies appeared to be identical — Stapel promised to be more careful in the future. As Zeelenberg pressed him, Stapel grew increasingly agitated.

Finally, Zeelenberg said: “I have to ask you if you’re faking data.”

“No, that’s ridiculous,” Stapel replied. “Of course not.”

That weekend, Zeelenberg relayed the allegations to the university rector, a law professor named Philip Eijlander, who often played tennis with Stapel. After a brief meeting on Sunday, Eijlander invited Stapel to come by his house on Tuesday morning. Sitting in Eijlander’s living room, Stapel mounted what Eijlander described to me as a spirited defense, highlighting his work as dean and characterizing his research methods as unusual. The conversation lasted about five hours. Then Eijlander politely escorted Stapel to the door but made it plain that he was not convinced of Stapel’s innocence.

And yet as part of a graduate seminar he taught on research ethics, Stapel would ask his students to dig back into their own research and look for things that might have been unethical. “They got back with terrible lapses­,” he told me. “No informed consent, no debriefing of subjects, then of course in data analysis, looking only at some data and not all the data.” He didn’t see the same problems in his own work, he said, because there were no real data to contend with.

Replicate This

Do classic psychological studies published in high-profile journals hold up? The Reproducibility Project aims to find out.

There are few psychological effects better known—or more widely accepted—in academic halls than what is called semantic priming. Show a person a simple stimulus, something as unremarkable as a photograph of a cat. Let some time pass, then ask that same person to list as many words as possible that start with the letter c. This person is more likely not only to come up with the word cat, but to mention catlike animals such as cougars and cheetahs, because he was initially primed with that one little kitty cat.

Priming’s reach, of course, stretches far beyond cognitive tests. Therapists use it to help treat patients with depression during therapy sessions. Advertisers count on commercials to prime us to buy key brands during our trips to the mall or the grocery store. Priming is considered an underlying mechanism in stereotyping. And the word has become part of our cultural lexicon, too. We talk about how we are “primed” to feel, to want, to need, to talk. Priming is everywhere.

And yet, many of the classic studies that led us to our current understanding of priming have never been replicated. In fact, the few attempts to reproduce the results that we have taken at face value for so long have failed. In late 2012, that led Daniel Kahneman, noted Princeton University psychologist and author of the best-selling book Thinking Fast and Slow, to write an open e-mail to the entire priming-research community. He wrote, “Your field is now the poster child for doubts about the integrity of psychological research. Your problem is not with the few people who have actively challenged the validity of some priming results. It is with the much larger population of colleagues who in the past accepted your surprising results as facts when they were published.” Kahneman’s solution? A new research protocol whereby cooperating labs attempt to check and replicate each other’s studies. This is the only way, he argues, to separate the scientific wheat from the chaff.

Ethics of 2 cancer studies questioned

India studies funded by Gates Foundation, National Cancer Institute draw scrutiny

For more than 12 years, as part of two massive U.S-funded studies in India, researchers tracked a large group of women for cervical cancer but didn’t screen them, instead monitoring them as their cancers progressed. At least 79 of the women died.

One study, funded by the National Cancer Institute, did not adequately inform more than 76,000 women taking part about their alternatives for getting cervical-cancer screening; and those women did not give adequate informed consent, according to the Office of Human Research Protection, part of the U.S. Department of Health and Human Services.

The other study, funded by the Gates Foundation, is under review by the Food and Drug Administration, according to Kristina Borror, the OHRP’s director of compliance oversight. That study has raised similar concerns regarding 31,000 women who were tracked but not routinely screened or treated for cervical cancer.

Both studies continue today, though researchers for both told The Arizona Republic they have begun to offer screening to the women.

While the two studies differ in important respects, both included large numbers of women placed in “control groups” who were offered free visits with health-care workers, but who, until recently,were not screened for cervical cancer. Instead, researchers met with and tracked these women to monitor how many would develop cervical cancer and die, so their death rates could be compared with those of women who were being screened and treated for free.

Catastrophic neglect of basic sciences in medicine

Inappropriate attention to basic sciences could also unduly delay implementation of such simple interventions, resulting in unnecessary death and suffering. The conceit is not that “epidemiology is the basic science of clinical medicine”. The real conceit is to believe that health issues are mainly solved by basic scientists and health professionals. The truth, although unpalatable to some, is that health-related problems are mostly the result of the social and physical environment during our life course.....
