The therapist's dilemma: Tell the whole truth?

Abstract

Honest communication between therapist and client is foundational to good psychotherapy. However, while past research has focused on client honesty, the topic of therapist honesty remains almost entirely untouched. Our lab's research seeks to explore the role of therapist honesty, how and why therapists make decisions about when to be completely honest with clients (and when to abstain from telling the whole truth), and the perceived consequences of these decisions. This article reviews findings from our preliminary research, presents a case study of the author's honest disclosure dilemma, and discusses the role of therapeutic tact and its function in the therapeutic process.

Based on our preliminary research, one of the most common topics of overt dishonesty among therapists was their feelings of frustration or disappointment toward their clients. For example, a therapist working with a client with a diagnosis of avoidant personality disorder may find herself increasingly frustrated by the client’s continual resistance to discussing emotional topics or engaging in activities that would broaden his or her world. Such a client —let’s assume male—is also likely to feel preoccupied with concerns about whether the therapist “likes” him or feels as frustrated with him as he does with himself. Should this client apologize for his behavior and ask if the therapist is frustrated with him, the therapist may feel compelled to reduce the discomfort he is already experiencing by dispelling his concern: “No, it’s okay, I’m not frustrated.”

But either at this moment or at a later point in therapy, once rapport (i.e., the therapeutic alliance) has been more firmly established, a more honest answer to this question might be fruitful: “Yes, I am feeling frustrated that we haven’t been able to find ways for you to implement the changes we discuss here, outside of session. How does it feel for you to hear that I am feeling frustrated?” Or, arguably, an even more honest answer: “Yes, I am sometimes frustrated. I sometimes think we could go deeper here—I think it’d be helpful.” Or, an honest answer that is somewhat less critical of the patient and more self‐focused: “I do feel frustrated that I haven’t been able to be more helpful.” Clearly, there are many ways for a therapist to be honest and/or dishonest, and there are also gradations in whichever direction a therapist chooses.

Empirical Work in Moral Psychology

Abstract

How do we form our moral judgments, and how do they influence behaviour? What ultimately motivates kind versus malicious action? Moral psychology is the interdisciplinary study of such questions about the mental lives of moral agents, including moral thought, feeling, reasoning and motivation. While these questions can be studied solely from the armchair or using only empirical tools, researchers in various disciplines, from biology to neuroscience to philosophy, can address them in tandem. Some key topics in this respect revolve around moral cognition and motivation, such as moral responsibility, altruism, the structure of moral motivation, weakness of will, and moral intuitions. Of course there are other important topics as well, including emotions, character, moral development, self-deception, addiction, well-being, and the evolution of moral capacities.

Some of the primary objects of study in moral psychology are the processes driving moral action. For example, we think of ourselves as possessing free will, as being responsible for what we do; as capable of self-control; and as capable of genuine concern for the welfare of others. Such claims can be tested by empirical methods to some extent in at least two ways. First, we can determine what in fact our ordinary thinking is. While many philosophers investigate this through rigorous reflection on concepts, we can also use the empirical methods of the social sciences. Second, we can investigate empirically whether our ordinary thinking is correct or illusory. For example, we can check the empirical adequacy of philosophical theories, assessing directly any claims made about how we think, feel, and behave

Understanding the psychology of moral individuals is certainly interesting in its own right, but it also often has direct implications for other areas of ethics, such as metaethics and normative ethics. For instance, determining the role of reason versus sentiment in moral judgment and motivation can shed light on whether moral judgments are cognitive, and perhaps whether morality itself is in some sense objective. Similarly, evaluating moral theories, such as deontology and utilitarianism, often relies on intuitive judgments about what one ought to do in various hypothetical cases. Empirical research can again serve as an additional tool to determine what exactly our intuitions are and which psychological processes generate them, contributing to a rigorous evaluation of the warrant of moral intuitions.

Do psychotropic drugs enhance, or diminish, human agency?

Psychological medications such as Xanax, Ritalin and aspirin help to modify undesirable behaviours, thought patterns and the perception of pain. They purport to treat the underlying chemical cause rather than the social, interpersonal or psychodynamic causes of pathology. Self-knowledge gained by introspection and dialogue are no longer our primary means for modifying psychological states. By prescribing such medication, physicians are implicitly admitting that cognitive and behavioural training is insufficient and impractical, and that ‘the brain’, of which nonspecialists have little explicit understanding, is in fact the level where errors occur. Indeed, drugs are reliable and effective because they implement the findings of neuroscience and supplement (or in many cases substitute for) our humanist discourse about self-development and agency. In using such drugs, we become transhuman hybrid beings who build tools into the regulatory plant of the body.

Recreational drugs, on the other hand, are essentially hedonic tools that allow for stress-release and the diminishment of inhibition and sense of responsibility. Avenues of escape are reached through derangement of thought and perception; many find pleasure in this transcendence of quotidian experience and transgression of social norms. There is also a Dionysian, or spiritual, purpose to recreational inebriation, which can enable revelations that enhance intimacy and the emotional need for existential reflection. Here drugs act as portals into spiritual rituals and otherwise restricted metaphysical spaces. The practice of imbibing a sacred substance is as old as ascetic and mindfulness practices but, in our times, drugs are overwhelmingly the most commonly used tool for tending to this element of the human condition.

The Strange Order of Things: why feelings are the unstoppable force

“Feelings have not been given the credit they deserve as motives, monitors, and negotiators of human cultural endeavours.” In claiming simplicity, it is possible the author is being a mite disingenuous. The tone in which he sets out his argument is so carefully judged, so stylistically calm and scientifically collected, that most readers will be lulled into nodding agreement. Yet a moment’s thought will tell us that we conduct our lives largely in contradiction of his premise, and for the most part deal with each other, and even with ourselves, as if we were pure spirit accidentally and inconveniently shackled to half a hundredweight or so of forked flesh.

“Feelings, and more generally affect of any sort and strength,” Damasio writes, “are the unrecognised presences at the cultural conference table.” According to him, the conference began among the bacteria, which – who? – even in their “unminded existence … assume what can only be called a sort of ‘moral attitude’”. In support of his claim, he adduces the various ways in which bacteria behave that bear a striking resemblance to human social organisation. The implication is, then, that “the human unconscious literally goes back to early life-forms, deeper and further than Freud or Jung ever dreamed of”. Damasio’s argument is that we are directly descended not only from the apes, but from the earliest wrigglers at the bottom of the primordial rock pool.

The keyword throughout the book is homeostasis, of which he offers a number of definitions, the clearest of which is the earliest, and which he favours enough to set it in italics: homeostasis is the force – the word seems justified – that ensures that “life is regulated within a range that is not just compatible with survival but also conducive to flourishing, to a projection of life into the future of an organism or a species”.

There’s a Right Way and a Wrong Way to Do Empathy

When we talk about empathy, we tend to talk about it as an unqualified good thing. Research has shown that empathy is associated with kindness and helping behaviors, while its absence, clinically referred to as psychopathy, is associated with manipulation and criminal deviance. Empathy, some scientists have concluded, allows us to function well with others and survive as a species.

But what people often don’t talk about is how even a good thing like empathy can still be emotionally draining. Empathic people who easily take on other people’s feelings can spend their days feeling overwhelmed, hurt, and heavyhearted. Empathy, in other words, can be downright stressful. So would it be fair to say that sometimes it’s unhealthy?

A paper published earlier this month in the Journal of Experimental Psychology set out to answer exactly that. According to the authors, there are “two routes” to empathy. The first is imagining how someone else might feel in a given circumstance, called “imagine-other-perspective-taking,” or IOPT. The second is actually imagining yourself in the other person’s situation, called “imagine-self-perspective-taking,” or ISPT. With IOPT, you acknowledge another person’s feelings; with ISPT, you take on that person’s feelings as your own.

A Short History of Empathy

In the past few decades, interest in empathy has spread beyond psychology to primatology and neuroscience. In the 1990s, neuroscientists studying monkeys discovered mirror neurons, cells in the animals’ brains that fired not only when a monkey moved, but also when the monkey saw another one make the same movement. The discovery of mirror neurons spurred a wave of research into empathy and brain activity that quickly extended to humans as well. Other recent studies have further widened empathy’s reach into fields like economics and literature, finding that wealth disparities weaken empathic response and that reading fiction can improve it.

But as Kristof and Bloom illustrate, there is still some cultural debate about what empathy means today. And in the psychology community, the answers are no more clear-cut. Critics of the mirror-neuron theory, for example, question not only the location of these neurons in the human brain, but whether simulation of another’s gestures is a good description of empathy in the first place. The social psychologist C. Daniel Batson, who has researched empathy for decades, argues that the term can now refer to eight different concepts: knowing another’s thoughts and feelings; imagining another’s thoughts and feelings; adopting the posture of another; actually feeling as another does; imagining how one would feel or think in another’s place; feeling distress at another’s suffering; feeling for another’s suffering, sometimes called pity or compassion; and projecting oneself into another’s situation.

Q: One of the themes that comes through is that we feel so sure about the origins and motivations of various sexual behaviors, and for a good many of them there’s no scientific basis for feeling this way – indeed, in many cases science is far from reaching a conclusion. Why do you think we’re so prone to staunchly believing that how we feel about a sexual behavior is automatically true?

A: It’s certainly one of those areas where everyone has an opinion. But if there’s one thing I discovered while working on this book, it’s that the strength of one’s moral convictions about sex usually reflects the depths of one’s ignorance about the science of sex. The more one learns in this area, paradoxically, the more uncertain one becomes.

Human beings are “stomach philosophers”—we allow our gut feelings to make decisions about other people’s sex lives on the basis of whether or not we’re personally disgusted or uncomfortable with their erotic desires or behaviors. I draw the line at harm, but defining harm can be a slippery matter, too. Since we would be harmed, we presume that others must be harmed as well, even when that’s far from apparent. I joke in the book about how I’d be irreparably damaged if Kate Upton were to pin me to my chair and do a slow strip tease on my lap. Lovely as she is, I’m gay, and not only would I not enjoy that experience, I’d be made deeply uncomfortable by it. My straight brother or my lesbian cousin, by contrast, would process this identical Upton event very differently.
